About Us

Jan has lived in the Chicagoland area for over 25 years and has worked in senior living and real estate much of this time.  Having gone through the process of finding the right care for her parents and watching others as they struggled through this most difficult task, she realized there was a serious need for support for seniors transitioning.  Her passion is to help families through this very stressful time.  The search for home care or for an appropriate senior living community is time consuming, frustrating and confusing.  Jan is very familiar with the options in housing and home services needed to streamline the search.  She provides the best options for families to choose from for a healthier lifestyle for their loved ones.   It is Senior Transitions Concierge that does the legwork, researching  the options and providing the best for the adult influencer to evaluate.  She helps guide clients through the maze of options, connecting them to reputable, reliable service providers who will help their loved ones age in grace. 

Jen Walenga, LSW, has worked in healthcare for 10 years in various settings as a social worker, senior living advisor and liaison in healthcare & marketing. She has an undergraduate degree in marketing and a master’s degree in social work, and is a licensed social worker and licensed health insurance agent.

Jen has clinical experience working within the hospital, skilled nursing facilities and home health environment, helping patients and families advocate and develop care plans. Her background allows for an easy understanding and compassion towards senior transitions. Jen cares about helping the client all the way through the entire process while providing the support needed to make good decisions.

Aside from business, Jen enjoys running races (5 marathons under her belt), walking her dog, Pistachio, and being a wife and mother to two step sons.
